By Papiya Halder: – A team of doctors at Fortis Hospital, Anandpur, Kolkata successfully conducted complex Bentall Surgery on a 38 year old man recently. The patient was born with Congenital Heart Defect known as Bicuspid Aortic Valve. The team that did the surgery was led by Dr. K. M. Mandana, Senior Consultant Cardiothoracic & Vascular Surgery, Fortis Hospital, Anandapur.
The patient, Mr. Mrinal Kanti Roy, was suffering from this congenital deformity where he had only two cusps instead of three in his aortic valve in the heart. As a working professional in Jamshedpur, Mr Roy started showing symptoms for a partially occluded (diseased) Aortic Valve which is known as Aortic Stenosis. In due course of time, if the condition remains untreated, then a bicuspid aortic valve may be calcified narrowing the valve. This results restricted blood flow through the valve.
In 2013, an Aortic Valve Replacement surgery was done at a different hospital to address his condition. However, the discomforting feeling relapsed after six months of the surgery. After a second consultation with his cardiologist, he underwent similar surgery. However, the condition degenerated yet again.
The patient eventually consulted Dr K M Mandana as his symptoms worsens. The patient was thoroughly examined. The results of confirmed a situation known as stuck Aortic Valve Leaflet (Metallic prosthesis AVR). The lower end of Aorta, also known as Aortic Root, showed a narrowing passage along with stuck aortic valve prosthesis.
According to Dr. K M Mandana, Senior Consultant Cardiothoracic & Vascular Surgery, Fortis Hospital, Anandapur, “The only possible option was to perform a Redo Aortic Valve Bentall surgery. This would clear the obstruction at the Aortic Valve area and allow us to put a new valve with a new design to function safely in the Aortic Root. The healed tough tissue obstructing the coronary flow was corrected.”
The unique yet complex Bentall surgery was evaluated as a possible measure to address the problem. The operation involves composite graft replacement of the aortic valve, aortic root and ascending aorta in addition to re-implantation of coronary arteries into the graft.
“The Redo Aortic Valve Bentall surgery is a highly complex surgery. We conducted the operation to replace the Aortic annulus and Valve. All the fibrosed tube around his coronary arteries were excised and repositioned. The defect has been corrected & Mr Roy will be able to lead a normal life”, Dr Mandana further added.
It’s been two and a half months since the surgery. Mr. Roy returned to Jamshedpur to resume work & his day to day activities.
