In the last one month, a couple of unique and life saving interventional procedures have been done by the dedicated Interventional Pulmonology team of Ruby General Hospital comprising of Dr. Subhankar Chakraborty, Dr. Deboprasad Das and Dr. Riksoam Chatterjee. These procedures are extremely rare in Eastern India and have turned the Life Threatening situations to Instant Relief for the patients without any cut. We would like to discuss these couple of unique interventional procedures which have helped in excellent patient outcome.

The first case is about a 62 year old lady from Jharkhand who had initially been presented in OPD with complaints of progressive respiratory distress, dry cough with streaky hemoptysis and left sided severe chest pain for last three months and then rushed for admission through Emergency as she was unable to even breathe while waiting in the OPD. After a thorough clinico radiological evaluation videobronchoscopy was performed which revealed a bulky tumor growth fully obstructing the left main bronchus causing complete collapse of left lung. After discussion with the family members and with written and informed consent the lady had been posted for rigid bronchoscopy under general anaesthesia.

The tumour was completely removed by the expert team of trained Pulmonologists very carefully, restoring the airway. Post procedure the left main bronchus reopened completely, the left lung expanded fully, giving instant relief to the patient. This would not have happened if the lung remained collapsed for a longer period. Rigid Bronchoscopy helped make this procedure possible. Rigid bronchoscopy is a minimally invasive, operative procedure to visualize and treat central airway obstructions, such as tumours, foreign bodies, or strictures. This is primarily used for therapeutic interventions, including removing foreign bodies, debulking tumours, managing life-threatening hemoptysis, and placing airway stents. The second case is about a 69 year old senior citizen from Midnapore who presented with complaints of shortness of breath and chest discomfort with a prior history of COPD. Being a heavy smoker, low dose CT thorax was done which revealed a spiculated nodule in the right upper lobe and pendunculated intrabronchial growth in Left upper lobe. Subsequent FDG Digital PET-CT scan showed significant SUV uptake, raising strong suspicion of malignancy. Bronchoscopic evaluation by the Interventional Pulmonology team revealed that the anterior segment of the left upper lobe bronchus was completely occluded by a sessile pedunculated endobronchial tumor. Using advanced interventional techniques, the entire tumor was successfully removed en bloc using a bronchoscopic snare with electrocautery. Adequate hemostasis was secured, resulting in complete recanalisation of the airway. Radial EBUS (catheter) guided biopsy was taken from Right upper lobe lung nodule. The patient tolerated the procedure well, remained stable post-procedure, and is currently doing well with significant symptomatic improvement. Again EBUS instrument made this procedure possible. Thanks to the technological advancements.
